A certified PWD of MAKATI

Last February 16, 2011 I already got my Person with Disability (PWD) cards from Makati City Hall. These are cards given to any peron with defect (Makati Residence) who is entitled of a 20% discount on medical and dental services, medicines, basic commodities, transportation, admission fees in all establishments and educational assistance. It actually took me 2 weeks to get it but if cards are available, processing of this will only take you a couple of days.

These are the PWD Cards of Makati City

PERSON with DISABILITY FREE MOVIES BOOKLET

Present this booklet personally to any Makati City Cinema Ticket Booth Teller together with your ORIGINAL PWD ID card to avail of free movies exclusively for Makati City registered PWD. NO BOOKLET, NO FREE TICKET. Xerox or scanned copy or any other facsimile of your PWD ID CARD is not allowed. The booklet is good only from the date it was issued until date as shown on page; Actually the booklet is valid only for a year.

You are entitled to watch as many free movies as you wish in any given day, but only one (1) ticket at a time (per movie) shall be issued. You have to return to the teller's booth for your ticket for the next movie you wish to watch;

You should surrender your expired or used booklet to MWSD to secure another booklet from with control number. In case of loss, report to MSWD for immediate replacement.

PURCHASE BOOKLET

This booklet should be brought along with your PWD ORIGINAL ID to avail and enjoy a special discount of five percent (5%) of the regular retail price of basic necessities and prime commodities listed under Section 2 (a) and (b) of this Order. The total amount of said purchase shall not exceed the amount of One Thousand Three Hundred Pesos (Php 1,300.00) per calendar week without carry over of the unused amount.

PURCHASE BOOKLET ON MEDICINES

This should be presented together with your PWD ORIGINAL ID. The use of this booklet shall be guided by the "Guidelines for the twenty percent (20%) discount in the purchase of medicine provided that it is supported by the Doctor's prescription.

Discount privileges shall be non-transferable and exclusive for the benefits of the PWD.
